Completing the Development Intake Assessment Form

Why is it important to fill out the lengthy Developmental Intake Form and send them back before the evaluation?

You Know Your Child Best

Since caregivers spend a great deal of time with their children, they know their child best. Therefore, it is important for the occupational therapist, speech-language pathologist, and physical therapist to understand how children engage with the environment from the caregiver’s perspective. Our therapists will review your comments, concerns and information before the evaluation to allow our therapists to set up the best testing environment before the evaluation. Additionally, the concerns listed by the caregivers are used for planning treatment goals.
